Foundation Request for Funds

We have three types of request for funds forms available for your use in transferring funds to the campus.

  1. Request for Funds Form – Recurring: This form initiates having all available cash transferred monthly to the Regents transfer fund.  Transfers will occur indefinitely until the STOP form below is received.  Endowed chair funds may request a recurring transfer, however these transfers must be renewed annually. 
    1. STOP Recurring Request for Funds Form:  This form is used to stop a recurring request for funds.
  2. Request for Funds Form – Non-RecurringThis form allows you to submit a request for a one-time withdrawal of money from one or multiple Foundation funds. If you’re requesting funds for a chair please use the Endowed Chair Form.
  3. Request for Funds Form – Endowed ChairThis form is to be used to submit a request for an endowed chair. As chair funds require approval from the Vice Chancellor or their delegate prior to processing, this form cannot be emailed. Please print this form and submit to the Vice Chancellor or their delegate for their signature and then submit to us by email, interoffice mail at MC0940 or fax at (858) 534-8160.
Request for Funds Form – Endowed Chair: This form is to be used to submit a request for an endowed chair. As chair funds require approval from the Vice Chancellor or their delegate prior to processing, this form cannot be emailed. Please print this form and submit to the Vice Chancellor or their delegate for their signature and then submit to us as directed below.

Foundation's W-9 Form

If you would like a copy of our form W9, please send an email with the business purpose of your request to UCSDFoundationAcctng@mail.ucsd.edu.   A Foundation Accounting staff member will respond to your request within two business days.
